A critical component at succeeding at value-based purchasing is improving performance on the HHCAHPS surveys. Yet, for many organizations, this can be the most difficult metric to change. In this session we'll share preliminary results of the NAHC sponsored Patient and Family Satisfaction Quality Improvement Project as well as national HHCAHPS trends from the largest CAHPS database for home health agencies. We'll also hear in an agency Project participant to discuss their own outcomes.

Value Based Purchasing will be changing in 2025 with the deletion and addition of measure that will be utilized to determine an agency's total performance score (TPS) and subsequent payment adjustment. This must-attend session will provide and overview of the changes that includes new measure calculations, preliminary provider data on the new and existing measures and strategies to improve outcomes that will be critical to providers as they move into success planning for VBP 2025.

Learning objectives:

  • Explain the 2025 VBP Measures including the sources of data and calculations of the measures making up the TPS.
  • Provide preview of performance data available for VBP 2024 measure results and new 2025 measure preliminary performance data.
  • Discuss targeted strategies to optimize performance in the new 2025 VBP model.
