HIS 3.02 to HOPE 1.00 Crosswalk Guide
Per CMS, the new HOPE assessment will replace the HIS assessment on October 1st, 2025.
With the HOPE assessment going into effect on October 1st, 2025, SHP is pleased to provide hospice agencies with a complete side-by-side comparison of the HIS and HOPE assessment forms. Items that have been added or removed between the two assessments are indicated with color coding.
This document includes all items recorded at Admission, HUV, and Discharge. A check is in each column to correlate to the assessment reasons for which the item is recorded.
We recommend printing copies for your staff to aid in the transition to HIS to HOPE.
This guide is an excellent reference for anyone who works with the new HOPE Assessments and will improve accuracy, a quick guide to all the new items and the time points they are to be addressed.
